Pathways is a transitional living program that provides housing and support services to young adults ages 18-24.

 

   
 

Program Description

Pathways is a transitional living and supportive services program for young adults ages 18-22 years. The program serves homeless, and many of the participants are former foster care youth who have limited financial and emotional support.

 

If participants enter the program before their twenty-second birthday, they can stay in the program for up to eighteen months. Aftercare services are also available for any former client.

 

The program office and housing site is located at the southeast corner of Norwegian and Sunrise Avenue in Modesto. The apartments have three bedrooms and two bathrooms. Four participants live together in each apartment. Each apartment is equipped with laundry facilities.

 

There are no program fees; however, residents are held financially responsible for any damage or loss of Pathways property.
                                                    

All participants are required to work in the community in a volunteer capacity within 60 days of entrance into the program and be employed within 180 days of entrance. Participants are assisted with resume writing and job searches.

 

In addition, participants must attend all scheduled meetings and participate in the program ”individual savings plans”. Participants are required to save 50% of their income in an individual account with the program. When $2000 is saved, participants have the option of continuing to save money in their account or they can open an account outside the program. $2000 must be kept in their account until exiting the program.

Pathways provides the following services

  2. Weekly Case Management

  3. Weekly Substances Abuse Counseling

  4. Mentoring

  5. Mental Health Counseling

  6. Weekly groups

  7. Life Skills Classes

  8. After Care Services

  9. Assessments

  10. Food and Nutrition Education

  11. Recreation

  12. Supportive services

  13. Referral and linkage to the community

Pathways Orientation

Anyone interested in learning more about Pathways may attend a "Pathways Orientation " held every Tuesday

from 1:30 P.M. - 2:30 P.M. at 700 Norwegian Ave. Modesto, CA 95350

Participant responsibilities include:

  1. Attend all assigned classes and groups

  2. Maintain employment

  3. Meet with Case Manager weekly

  4. Meet with AOD counselor weekly (if indicated)

  5. Meet with counselor weekly

  6. Contribute to savings account

  7. Keep their apartment clean

  8. Complete yard work or other household assignements

  9. Abide by established rules and policies

Eligibility requirements

  1. Must be 18 to 21 years old (Youth must apply to the program before their 22nd birthday)

  2. Low Income

  3. Homeless

Documents required to become a Pathways Participant

  1. Birth Certificate

  2. California Identification or Student Identification with picture

  3. Social Security Card

  4. Letter of Homelessness - must include date youth became homeless and reason why

  5. Six months of pay stubs (if employed)

  6. Terms of Probation or Court Ordered

  7. Child Custody Order if participant is parenting
