Pathways is a transitional living and employment development program that provides housing and employment services to young adults ages 18-21.

 

   
       
 

Program Description

The Pathways Program is a transitional living, employment and supportive services program for young people ages 18-21 years. The Program serves homeless, low-income and former foster youth who have limited financial and emotional support.

 

Participants can stay in the program for up to eighteen months or until their twenty-second birthday. After care services are also available for any former client.

 

The Program is located at the southeast corner of Norwegian and Sunrise Avenue in Modesto. Each apartment has three bedrooms and two bathrooms. Four participants live together in each apartment. Each apartment is equipped laundry facilities.

 

There are no program fees; however, residents are held financially responsible for any damage or loss of Pathways property.                                                     

All participants are required to be employed within 120 days of enrolling in the program. Participants are supported to create resumes and search for jobs.

 

In addition, participants must attend all scheduled meetings and participate in the program’s “savings plan”.

 

Participants are required to save 50% of all their income in an individual account with the program. After a participant has saved $2000 dollars in their Pathways account they will have the option of continuing to save money in his/her account or they can open an account outside the program. They must leave the $2000 in their account until they leave the program.

Pathway’s provides the following services

  1. No Cost housing for up to eighteen months

  2. Weekly Case Management

  3. Weekly Substances Abuse Counseling

  4. Mentoring

  5. Mental Health Counseling

  6. Weekly groups

  7. Life Skills Classes

  8. After Care Services

  9. Assessments

  10. Food

  11. Recreation

  12. Supportive services

  13. Referral and linkage to the community

Participants are required to follow all rules and policies

  1. Attend all assigned classes and groups

  2. Maintain employment

  3. Meet with Case Manager weekly

  4. Meet with AOD counselor weekly

  5. Meet with counselor weekly

  6. Contribute to savings account

  7. Keep their apartment clean

  8. Do the yard work

Eligibility requirements

  1. Must be 18 to 21 years old

  2. Low Income

  3. Homeless

Requirements to apply for Pathways Program

  1. Birth Certificate

  2. California Identification or Student Identification with picture

  3. Social Security Card

  4. Letter of Homelessness

  5. Six months of work records (if employed)
