Notice of Privacy Practices

Center for Human Services is committed to protecting the privacy and integrity of our clients’ health information. The HIPAA Privacy Rule recognizes the importance and value of this commitment.

Federal regulations under the Health Insurance Portability and Accountability Act (HIPAA) require Center for Human Services to post our Notice of Privacy Practices on our Website and make the notice available electronically through the Website.

This Notice of Privacy Practices explains how Center for Human Services may use, provide and disclose your Protected Health Information (PHI) to others for treatment, payment, healthcare operations and for other purposes allowed or required by law. You will be given a copy of this notice at your initial visit with any of our staff.
